Enrollment & Forms

We are currently accepting children aged 6 weeks through 5 years old. Below is our Childcare Center Enrollment Form, and the Childcare Medical Forms. Please note, the medical form must be completed by a licensed Physician, Physician’s Assistant or Nurse Practitioner.

You may mail us the forms, bring them to us in person, or send them to us as an email attachment to childcare@trinityroslyn.org.
